Community First Services and Supports is a self-directed program in Minnesota. It gives individuals flexible options and choices, enabling them to take charge of their care. This promotes independence, allowing people to flourish in their homes and communities while receiving personalized support that addresses their specific needs.​

How to begin CFSS services: 

Initial assessment takes place at the beginning of the process to evaluate individual needs and establish a tailored service plan. This assessment helps to ensure that all services are identified for effective care. Regular annual reassessments are also conducted to adapt to any changes in needs over time.​

CFSS Service Delivery Model Options:

 

AGENCY PROVIDER MODEL and BUDGET MODEL (both provided by D'crimmon Care)

Each model is designed to offer different levels of responsibility for people who receive services. Both models empower individuals to select and obtain the services that best suit their needs.

IN BOTH MODELS:

You and/or your representative have the ability to:

• Direct your own care

• Have a say in selecting your worker(s), including the ability to hire a spouse or the parent of a minor, or even another person who uses CFSS services.

• Write your own service-delivery plan with help from your Consultation Services provider

• Buy goods and services, if desired

• Buy a personal emergency response system (PERS), if desired

 

AGENCY  MODEL:

If you choose the Agency Provider Model, you select an agency provider, like D'crimmon Care, to serve as your workers’ employer. As an agency, we will be responsible to recruit, hire, train, supervise and pay your CFSS support workers.

BUDGET MODEL:

If you choose budget model, you will be the employer of your support workers. You will have a budget rather than service units. As the employer, you will be responsible to recruit, hire, train and supervise your support workers. You will select a Financial Management Services (FMS) provider, to help you with your payroll and other employer related tasks.
